Food Intolerance And Allergy Testing

Some of the earliest work on food intolerance began in 1978, when a group of Australian researchers published details of an exclusion diet to exclude specific food chemicals from the diet of patients. This provided a basis for challenge with these additives and natural chemicals. In 1995 the European Academy of Allergology and Clinical Immunology suggested a classification on the basis of the responsible pathogenetic mechanism; according to this classification, non-toxic reactions can be divided into food allergies when they recognise immunological mechanisms, and food intolerances when there are no immunological implications. In 2003 the Nomenclature Review Committee of the World Allergy Organization issued a report for global use on food allergy and food intolerance, that has had general acceptance.

Food intolerance or non-allergic food hypersensitivity is an adverse reaction to some sort of food, beverage or food additive that occurs every time the food is eaten, but particularly if larger quantities are consumed and should not be confused with a food allergy. It can involve symptoms in one or more body organs and systems, but is not considered directly related to the immune system. Intolerance can result from the absence of specific chemicals or enzymes needed to digest a food substance, or from reactions to naturally occurring chemicals in foods.

It can be difficult to determine the offending food causing an intolerance reaction because if the immune system is involved, the response is likely to take place slowly. Thus the cause of the sensitivity and the response are separated in time, and may not be obviously related. Diagnosis is made using medical history and medical tests to exclude other causes, but to obtain final confirmation a Double Blind Controlled Food Challenge must be performed as it is important to be able to correctly diagnose between food allergy, food intolerance, and auto-immune disease in the management of these disorders. In order to diagnose food intolerance an elimination diet will often be recommended designed to exclude foods likely to cause reactions and foods which commonly cause true allergies. It can take a period of days or weeks before symptoms subside, but when symptoms have reduced considerably then challenge testing can begin. This involves selected foods containing only one food chemical so as to isolate it if reactions occur. Once food sensitivities have been identified, a suitable diet can be prescribed.
